Buyer initials verify understanding and agreement to the following terms:
1. AKC Registration & Breeding Restriction
This puppy is sold with AKC Limited Registration, included in the purchase price.
Limited Registration means the dog is not to be used for breeding and any offspring will not be eligible for AKC registration.
Buyer agrees this dog will not be used for breeding at any time. The dog must be spayed or neutered. Breeder recommends delaying spay/neuter until approximately 18 months of age for developmental health reasons; timing is the responsibility of Buyer.
Use of this dog for breeding constitutes a material breach of contract. In such event, Buyer agrees to pay liquidated damages in the amount of $1,000.00, plus reasonable attorney’s fees and court costs incurred in enforcement.
Initial ______
2. 72-Hour Health Guarantee
Seller certifies this puppy is a purebred Siberian Husky from an AKC-registered litter and is in good health at time of transfer.
Buyer must have the puppy examined by a licensed veterinarian within seventy-two (72) hours of possession (excluding Sundays and federal holidays) for this guarantee to remain valid.
If a serious health condition is identified within this period:
• Seller must be notified immediately.
• Written documentation from the examining veterinarian must be provided.
• If death occurs within the 72-hour period, a necropsy performed by a licensed veterinarian is required to determine cause of death.
If these conditions are met, Seller will provide a replacement puppy of the same sex from the next available comparable litter. No monetary refunds or reimbursement of veterinary expenses will be provided.
Initial ______
3. 24-Month Genetic Health Guarantee (Hips & Eyes)
This puppy is covered by a twenty-four (24) month genetic health guarantee for:
• Genetic Hip Dysplasia (confirmed by OFA or PennHIP)
• Juvenile Cataracts
• Progressive Retinal Atrophy (PRA)
Buyer must provide:
• Official OFA/PennHIP documentation (for hips)
• Certification from a board-certified veterinary ophthalmologist (for eyes)
• Proof of 72-hour veterinary exam
• Documentation that the dog has not been abused, neglected, injured, or bred
Upon verification, Buyer will receive a replacement puppy from the next available comparable litter. No cash refunds will be issued.
Initial ______
4. Supplements

Initial ______
Lifetime Return Policy
If Buyer is unable to keep the dog at any time during its life, the dog may not be surrendered to a shelter or rescue. Placement with a third party will involve Breeder’s written consent. Return will be for breeder to find appropriate home and does not involve a refund. Re-homing fees typically paid past puppyhood are minimal and this is about finding the best placement for the puppy. When purchasing a puppy, intent should be for a lifetime commitment.
Violation of this clause constitutes a material breach of contract. Buyer agrees to pay liquidated damages in the amount of $1,000.00 plus reasonable attorney’s fees and court costs.
Return of the dog does not entitle Buyer to a refund.
Initial ______
